Dear Parents:

While the school year has just ended, we are already making plans for the new academic year. Please take a few moments to read through this information carefully and share it with your child.

Summer Reading and Books

The summer reading list provided here describes the selections for all students entering the Bolles Middle School Bartram Campus.

Text Books

All textbooks required by the School are available online through MBS Direct here.

Dress Code Information

There is a prescribed dress code policy at Bolles Middle School Bartram campus. If you have any questions about the information provided here, please call the Middle School Office.

Immunizations and Health Forms 

There are a few medical forms that need to be supplied to the school:

REQUIRED REPLY, OBTAIN FORM FROM YOUR DOCTOR Immunizations (new students only) We are required by law to have a record of every student’s immunizations. Please have your child's physician fill out the Florida Certificate of Immunization form and deliver or mail it to the Bartram Campus Health Center. The form is obtained from your doctor’s office.

REQUIRED REPLY, OBTAIN FORM FROM YOUR DOCTOR ALL 7th graders All 7th graders in the State of Florida are required by law to have a series of inoculations with completed records on file at the student’s school. Please see your family physician for the proper form and to receive any inoculations that may be necessary. 7th graders my not start classes until the inoculations are up to date and the form is on file in the School office.

REQUIRED REPLY The consent for Emergency Health Care form provided here should be filled out EACH YEAR by all parents and returned to the Registrar’s Office at The Bolles School no later than August 1, 2012 (last year's form is not valid).  This form is our “hot sheet” and contains the student’s emergency contact information, medications that may be administered to your child, the actual Consent for Emergency Health Care, and important insurance information.  Please download the form here, and use black ink when completing this form. Please return this form BY AUGUST 1, 2012 to the Bartram Campus Health Center.

The Bolles School is required by HIPAA (the Federal Health Insurance Portability and Accountability Act) to preserve the privacy of your child’s health information.  In accordance with this policy, access to all student health forms is limited to the School nurse or administrative staff, and is only utilized for the safety and protection of your child or in an emergency.  By signing this form, you authorize The Bolles School to use your child’s health information only in the manner for which it was intended.


Athletics Information

Our fall sports calendar at the Bolles Middle School Bartram Campus is listed below and the 2012-13 FHSAA Sports Calendar can be viewed here.

REQUIRED REPLY I hope many of you plan on participating in one of our programs. Please make sure you have completed Physical, and Consent and Release Forms on record with us before your first day of practice or try-out. The newly-revised Consent and Release forms can be downloaded here. No one will be allowed to participate without these forms on file.

FOOTBALL – Coach Tom Collins
Practice will begin Monday, August 13, on the Bartram Campus and will be held every weekday (Monday-Friday) from 5 – 7 p.m. until the first day of school. If you have any questions call Coach Collins at 732-5703 or e-mail collinst@bolles.org.

GIRLS’ VOLLEYBALL GRADE 7 & 8– Coach Natalie Blackwell
Try-outs will be held Tuesday, August 21, through Friday, August 24, from 4 – 5:30 p.m. on the Bolles Middle School Bartram Campus. If you have any questions please e-mail Coach Blackwell at blackwell491@bellsouth.net.

Student Drop-off and Pick-up Safety Rules

1. Students should be dropped off and picked up ONLY at the designated area next to Parking Lot A (see campus map). Please do not drop off or pick up at the art room, the Childress House, Parking Lot B, or other locations.

2. Students may not cross the street at the pickup area. Parents should not park in Lot A and expect their students to cross through traffic to reach the car. Students must be picked up along the sidewalk only. Exceptions to this rule can be made occasionally to meet appointments etc. In this case the parent must personally escort their child across the street. This should be an occasional situation, NOT a daily occurrence.

3. The traffic circle is two lanes and is one-way counter clockwise. The right hand lane is for dropping off and picking up students. The left hand lane is for exiting the circle. Cars may not stop in the left hand lane. Please do not drop off or pick up students in the left hand lane.

4. Students may not exit on the driver’s side of the car. All students must exit on the passenger side of the car only.

Late arrival and early dismissal procedure
Between the hours of 9 a.m. and 3 p.m. parents should use the circle in front of the office for pickup and drop-off. Please do not ask your child to meet you by the Athletic Center for pickup between 9 and 3. This area in front of the office is reserved for buses before 9 a.m. and after 3 p.m. Please help us as we work to maintain a safe campus!

As you can imagine, scheduling classes for each of our students is a very long and complicated process. Look for your child’s schedule in the mail in early August.

Thank you for your attention to these materials. They are very important in assuring a successful fall semester for you and your child. I will be on campus for most of the summer and will be happy to assist you in any way.
